In consideration of being permitted to either observe or participate in the Jang Mu Won Hapkido International Association, I declare that I am both mentally fit and physically fit to have engaged in the proper training to participate in or observe the Jang Mu Won Hapkido Association / Jang Moo Won Hapkido Association / Jang Mu Won Kim’s Hapkido classes and/or instructor training courses. It is the member responsibility to consult with a physician prior to participating in any and all of the association’s martial arts and exercise classes, programs and/or workshops. Member represents that he/she will not use the facility with any infections or maladies with the potential harm to others or otherwise in accordance with public health requirements. If allowed to participate in or observe any seminar, I will abide by the rules used by Jang Mu Won Hapkido International , Jang Mu Won Hapkido Association, Jang Moo Won Hapkido Association, Jang Mu Won Kim’s Hapkido Association, Mr. Han W. Kim, Mr. Steven Petermann, Kim’s Hapkido (Alhambra, South Pasadena, Yucaipa, Arcadia and Forest Falls), affiliated school , their respective instructors (per the schools mentioned above), and Grandmaster Chong S Kim. I will also follow any instructions given by the aforementioned instructors or institutions. I also understand that if I or anyone involved directly or indirectly with me shows disrespect to any of the instructors or fails to follow directions/instructions issued by any of the instructors or institutions, I may be evicted from the class without refund of monies paid to participate or observe. I/we hereby waive any compensation whatsoever, for any pictures, films, movies, social media coverage and media coverage, etc. taken of me at these classes, and may be used by the promoters and those designated by the promoters now and in the future. I/we the undersigned, for himself/herself, personal representatives, heirs, next of kin acknowledges and agrees to the terms listed below:
Hereby releases, waives, discharges and covenants not to sue Jang Mu Won Hapkido International, Kim’s Hapkido Jang Mu Won, Jang Moo Won Hapkido Association, Jang Mu Won Hapkido Association, Kim’s Hapkido , Mr. Han W. Kim, Mr. Steve Petermann, Mr. Raymond Bouchereau, Mr. Jose Esparza, Mr. Gregory Castle, Kim’s Hapkido Alhambra, Kim’s Hapkido South Pasadena, Kim’s Hapkido Yucaipa, Kim’s Hapkido Arcadia, Kim’s Hapkido Forest Falls, instructors associated with the schools mentioned above, other participants such as students, immediate family members, and observers, sponsors, advertisers, the promoters of specified events, owners and lessees of the premises used and anyone directly or indirectly related to the event, now or in the future, all for the purpose here in referred to as “releases,” from liability to the undersigned, for any and all damages, and any claims or demands therefore on account of injury to the person or property or resulting in death of the undersigned, whether caused by the negligence of the releases or otherwise while the undersigned in participating in, observing and/or for any purpose participating in the event. The parties agree that special workshops, required uniforms, patches, required equipment, and other school/organization related activities are not covered within this document and remain the responsibility of the member. You agree also to further release the school/organization from any liability for any loss of theft or personal property.
Each of the undersigned expressly acknowledges and agrees that the activities at the event are dangerous and involve risk of serious injury and/or death and/or property damage. Each of the undersigned further expressly agrees that the foregoing release, waiver and indemnity agreement is extended to be as broad and inclusive as is permitted by the law of the province or state in which the event is conducted and that if any portion thereof is held invalid, it is agreed that the balance shall not withstanding continue in full legal force and effect.
The undersigned has read and understands the content thereof and voluntarily signs the release Wavier of Liability and Indemnity Agreement, and further agrees that no oral representations, statements or inducement apart from the foregoing written agreement have been made.
